Patient History Forms
Due to the nature of the tests performed by this laboratory, it is essential for the Patient History Form to be completed and sent with the blood samples to ensure that our staff has the information necessary to perform the appropriate antibody/antigen tests for each patient.

5cc Citrate (blue top)

20,000 to 49,999 30cc
10,000 to 19,999 40cc
Less than 10,000 *Do not draw

*If the platelet count is less than 10,000 we cannot recover sufficient platelets to perform the autoantibody test (Direct). Please contact the Lab Supervisor for your testing options.

Platelet Typing for Single Platelet Antigen 24 hours 10cc EDTA (lavender top) or 10cc ACD (light yellow top) Always send whole blood, room temperature.
