Platelet Alloantibody Workup (Basic)

Multiply transfused patients may become alloimmunized to foreign donors. Patients who have failed to respond to two consecutive random donor platelet transfusions may require HLA-selected platelet transfusions. Contact the Apheresis Donor Coordinator (425-453-5098) to schedule HLA-selected community donors. The Apheresis Scheduling Coordinator will ask you to send a blood sample from the patient to the Platelet Immunology Laboratory. HLA antibody tests will be used to detect alloantibodies.

In antibody-negative patients, HLA matched platelet transfusions are not likely to improve transfusion responses as these patients are probably receiving drugs or have clinical factor(s) that prevent them from responding to platelets from any source. Such patients should be returned to random donor platelet transfusion therapy.

In antibody-positive patients who require HLA-selected donors must be HLA-typed. If the patient has not been HLA-typed, contact the Immunogenetics Laboratory (206-292-1860) for sample requirements.
