ABO Minor Mismatch BMT Recipient Evaluation

This profile can be ordered for a patient who has received an allogeneic bone marrow transplant that involved a minor ABO mismatch. The appropriate time to order this test is starting approximately one-week status-post transplantation. Testing is routinely continued from Day 5 through Day 12 posttransplant. This test can be used as supportive evidence of hemolysis due to isoagglutinin production by the newly transplanted lymphocytes.

Testing includes a direct antiglobulin test. If positive, an initial eluate with red cell panel will be performed to confirm that the expected isoagglutinin is detected. IgM and IgG isoagglutinin titers (anti-A titer, anti-B titer) will be done if indicated. Percent of the recipient’s original red cells remaining in the sample will also be determined and may be followed with daily new patient samples, if indicated.

  • Normal Test Values: Dependent on donor and recipient ABO types and time from transplant.
  • Analytic Time: 24 hours
