This profile is indicated when a patient has an identified RBC antibody and crossmatch compatible RBCs are needed for transfusion. (Regional laboratories capable of performing the crossmatch may choose to order antigen negative RBCs.) The minimum testing performed for this profile will be ABO and D typing, an antibody screen and crossmatch with patient sample, and confirmation of RBC unit antigen status. Testing may also include antibody identification/confirmation, patient phenotyping, absorption, elutions or antibody inhibitions depending on the complexity of the case. These tests would only be required if the patient’s antibody was previously unidentified, the patient has been transfused or pregnant in the last 3 months or the expected reactivity has changed since the patient was last tested. Communication with the submitting facility may be requested in order to determine the scope of testing required and/or confirm the patient’s history.
